Refer A Child

Anyone between the age of 5- 21 who has relapsed from cancer could be eligible to receive a wish.

Referrals are accepted from the following sources:

• Medical professionals treating the patient including doctors, nurses, social workers, or child-life specialists
• Parent or legal guardian of the potential wish recipient
• The potential wish recipient

Once the referral is received, the treating physician will be contacted to determine whether the individual is medically eligible for a wish.

All medical information is considered confidential and is not discussed with outside parties unless it is required for the wish and the child’s parent or guardian have given consent.
